At Worthy Wellness and Aesthetics, patient safety, education, and informed decision-making are important components of our medical weight loss services. We believe patients should have a clear understanding of program expectations, treatment options, potential risks, and the factors that contribute to long-term success. Our goal is to provide evidence-based guidance, personalized support, and medically supervised care while helping patients make informed decisions about their health and wellness journey.
Our medical weight loss program is overseen by Chelsey Lossen, APRN, who provides individualized evaluations, treatment recommendations, ongoing monitoring, and support tailored to each patient's unique health needs and goals. We are committed to promoting safe, sustainable weight management through a comprehensive approach that emphasizes healthy lifestyle changes, patient education, and long-term wellness.
The following information is provided in accordance with Florida requirements for weight loss programs and should be carefully reviewed before participating in any medical weight management program.
Florida Required Weight Loss Program Disclosure
(A) Warning: Rapid Weight Loss May Cause Serious Health Problems. Rapid Weight Loss Is Weight Loss of More than 1-1/2 Pounds to 2 Pounds per Week or Weight Loss of More than 1 Percent of Body Weight per Week after the Second Week of Participation in a Weight-loss Program.
(B) Consult Your Personal Physician Before Starting Any Weight-loss Program.
(C) Only Permanent Lifestyle Changes, Such as Making Healthful Food Choices and Increasing Physical Activity, Promote Long-term Weight Loss.
(D) Qualifications of this Provider Are Available upon Request.
Your Rights as a Participant in a Weight Loss Program
As a participant in a weight loss program, you have the right to receive information about the services being provided and to make informed decisions regarding your care. Patients have the right to understand the program, ask questions, and receive transparent information regarding treatment recommendations, costs, and program expectations.
1. Ask Questions about the Potential Health Risks of this Program and its Nutritional Content, Psychological Support, and Educational Components.
Patients are encouraged to actively participate in their care by discussing any concerns, asking questions, and seeking clarification regarding the recommendations included in their personalized weight management plan. Understanding the nutritional, educational, behavioral, and wellness components of the program can help support informed decision-making and long-term success.
2. Receive an Itemized Statement of the Actual or Estimated Price of the Weight-loss Program, Including Extra Products, Services, Supplements, Examinations, and Laboratory Tests.
Patients have the right to receive clear information regarding the actual or estimated costs associated with the program. This includes fees related to consultations, follow-up visits, medications, laboratory testing, supplements, products, and any additional services that may be recommended as part of an individualized treatment plan.
3. Know the Actual or Estimated Duration of the Program.
Weight loss and weight management goals vary from person to person. Patients have the right to receive information regarding the anticipated duration of their program and discuss realistic expectations for progress based on their individual health history, treatment goals, and provider recommendations.
4. Know the Name, Address, and Qualifications of the Dietitian or Nutritionist Who Has Reviewed and Approved the Weight-loss Program According to S. 468.505(1)(J), Florida Statutes.
Patients may request information regarding the licensed dietitian or nutrition professional responsible for reviewing and approving the weight loss program in accordance with applicable Florida law.
